Questionnaire for students to understand the situation after the earthquake (To parents / guardians)

We would like to extend our deepest sympathies to those affected by the 2016 Kumamoto earthquake. This time, we have decided to conduct a “Questionnaire to understand the situation after the earthquake” for the purpose of support to students in Kumamoto University.
This questionnaire is to understand the situation of students after the 2016 Kumamoto Earthquake on April 14-16.
Purposes of this questionnaire as following:
 
1. Have the 2016 Kumamoto Earthquake and related matters influenced on students any of physical, mental or financial problems? If so, we will try to find them as soon as possible and help those who wish our help by treatment and/or providing relevant information.
 
2. The collected information with this questionnaire will be used to understand the future effects on students, and to utilize as the valuable information for the similar disaster in the future.
 
We ask students to answer, only when they understand and agree the purposes of this questionnaire. Even though they do not agree to answer the questionnaire, it is possible to consult with experts whenever you have any physical, mental or earthquake related other problems. We will consider about supports for students after the earthquake and assist them in the best. Thank you for your understanding and cooperation. Furthermore, participants of this survey include minors as well. If parents/guardians does not want to use minor student’s answers for this questionnaire to earthquake related documents and academic research, please do not hesitate to inform us such volition. If we receive it, we do not use their answers for this survey. Even in that case, it is possible to consult with experts, and there is no any disadvantage in consultation. We will conduct similar surveys regularly in future, it is possible to inform us such volition anytime.
