The extension of the survey period of “Questionnaire for students to understand the situation after the earthquake (Request)”
We would like to extend our deepest sympathies to those affected by 2016 Kumamoto Earthquake.
We conduct a survey about influence on mental health and sleep habits by earthquake for all students for the purpose of the following two points in Health Care Center and Student Accessibility Office. Please answer, only when you understand and agree the purpose of this questionnaire.
Have the 2016 Kumamoto Earthquake and related matters influenced on students any of physical, mental or financial problems? If so, we wish to find them earlier and help those who wish our help by treatment and/or providing relevant information.
The collected information with this questionnaire will be used to understand the future effects on students, and to utilize as the valuable information for the similar disaster in the future.
